A USCIS civil surgeon in Jackson Heights, Queens
Most people applying for a green card from inside the United States (adjustment of status) need an immigration medical exam, recorded on Form I-693, Report of Immigration Medical Examination and Vaccination Record. Only a doctor designated by USCIS, called a civil surgeon, can do this exam and sign the form.
Dr. Ilyayeva is listed in the USCIS civil surgeon finder. To see her listing, search ZIP code 11372 in the USCIS Find a Civil Surgeon tool.
Our office is easy to reach from Elmhurst, Corona, East Elmhurst and Woodside. Take the 7 train to 90 St–Elmhurst Av (about a 4- to 5-minute walk) or Junction Blvd (about a 9-minute walk). Who needs an I-693 immigration medical exam?
- Adults and children applying for a green card (lawful permanent residence) from inside the U.S., using Form I-485
- Husbands, wives, parents and children in the same family, each with their own exam and form
- Refugees adjusting status, who usually need only the vaccination part of the exam
- Anyone USCIS has asked for a Form I-693. If USCIS or your lawyer sent you instructions, bring them or tell us when you call
If your visa interview is at a U.S. embassy or consulate abroad, your exam is done by a panel physician in that country instead.
What happens at the exam
Dr. Ilyayeva follows the CDC's current Technical Instructions for Civil Surgeons. What each person needs depends on their age and health history:
- Checking the applicant's identity with a passport or other government-issued photo ID
- A medical history and a physical exam
- Review of vaccine records, and any vaccines that are required and missing (see the table below)
- A tuberculosis (TB) blood test, called an IGRA, for everyone 2 years and older. A chest X-ray is needed only if the test is positive, there are signs of TB, or the person has known HIV infection. Children under 2 get a TB check through their history and exam
- A blood test for syphilis for ages 18 to 44, and a lab test for gonorrhea (usually a urine sample) for ages 18 to 24. People outside those ages are tested only if there's a medical reason
- The completed, signed Form I-693, given to you in a sealed envelope, plus a copy of your vaccine record to keep
Vaccines required for the green card medical exam, by age
The CDC requires only certain vaccines for the exam, based on the applicant's age on the day of the exam. If you're missing one, you get at least one dose at the visit; you don't have to finish the whole series first. A vaccine that isn't right for your age or health, or a flu shot outside flu season, is noted on the form instead.
| Vaccine | Required for ages |
|---|---|
| Diphtheria, tetanus and whooping cough (DTaP) | 2 months through 6 years |
| Tetanus, diphtheria and whooping cough booster (Tdap or Td) | 11 years and older (sometimes ages 7 to 10, depending on past doses) |
| Polio | 2 months and older. For adults, it depends on whether they had the childhood series |
| Measles, mumps and rubella (MMR) | 12 months and older, if born in 1957 or later |
| Rotavirus | 6 weeks through 8 months |
| Hib (Haemophilus influenzae type b) | 2 through 59 months |
| Hepatitis A | 12 months through 18 years |
| Hepatitis B | Birth through 59 years |
| Meningococcal (MenACWY) | 11 through 18 years |
| Chickenpox (varicella) | 12 months and older |
| Pneumococcal | 2 through 59 months, and 65 and older |
| Flu (influenza) | 6 months and older, every year, during flu season |

Picking up your sealed Form I-693
The TB blood test result usually comes back within a few days. Once the whole exam is complete, Dr. Ilyayeva signs Form I-693 and we give it to you in a sealed envelope. If you file by mail, don't open it. If you file online, follow USCIS's instructions to open and upload it, and keep the original. We'll call you when your form is ready.
Fees and booking
Each civil surgeon sets their own fee for immigration medical exams. Call (917) 832-6562 to book and to ask about the fee. Please tell us how many family members need an exam and their ages, so we can plan the visit.

What languages do you speak?
Dr. Ilyayeva speaks English, Spanish and Russian, so she can explain each step of the exam in your language.
How do I book an immigration medical exam?
Call (917) 832-6562. Immigration exams are booked by phone. Our hours: Monday, Wednesday and Thursday 10:00 AM–6:00 PM; Tuesday 9:00 AM–1:00 PM; Saturday 10:00 AM–3:30 PM. Closed Friday and Sunday.
How much does the immigration medical exam cost?
Each civil surgeon sets their own fee. Call (917) 832-6562 to ask about our fee before you book.
How many visits does it take?
Usually two. The exam, blood tests and any vaccines are done at the first visit. Dr. Ilyayeva can sign Form I-693 only after the whole exam is complete, including the TB blood test result, so you come back to pick up the sealed form. We'll tell you when it's ready.
Do I need a TB test or a chest X-ray?
Everyone 2 years and older needs a TB blood test (called an IGRA); a skin test isn't accepted for this age group. A chest X-ray is needed only if the blood test is positive, there are signs of TB, or the person has known HIV infection. Children under 2 get a TB check through their history and physical exam.
Can vaccines be given at the exam?
Yes. If you're missing a vaccine that's required for your age, you get at least one dose at the exam. You don't have to finish the whole series first. Bring every vaccine record you have so we don't repeat vaccines you already had.
I came to the U.S. as a refugee. Do I need the full exam?
Usually not. Under CDC rules, refugees adjusting status need only the vaccination part of the exam. Bring your I-94 arrival record and your vaccine records. People who were granted asylum need the full exam.
My visa interview is at a U.S. embassy abroad. Can I come to you?
No. If your interview is at a U.S. embassy or consulate outside the United States, the exam is done by a panel physician in that country, not by a civil surgeon in the U.S.
Can I open the sealed envelope?
If you file Form I-485 by mail, send Form I-693 in the sealed envelope, unopened. If you file Form I-485 online, USCIS tells you to open the envelope, upload the form, and keep the original form and envelope until your case is decided. See USCIS Form I-693.
How long is Form I-693 valid?
Under current USCIS policy, a Form I-693 signed on or after November 1, 2023 is valid only for the application it's filed with. Submit it together with your Form I-485. If that application is withdrawn or denied, you'll need a new exam. See the USCIS Policy Manual.
