Exams and paperwork for school entry, daycare, camp and sports teams.
Schools, daycares, camps and sports teams often require a recent physical exam and a completed form. We do these exams, update vaccines if needed, and complete the forms you bring. Often the school or sports form can be completed at your child's yearly well-child visit, so a separate visit may not be needed.
Who it's for
Children and teens starting or changing schools, entering daycare or camp, or joining a sports team. NYC public schools use the Child and Adolescent Health Examination Form (CH-205) for new students and certain grades.
What happens at the visit
- A full physical exam, including heart and lungs
- Height, weight, blood pressure, and vision and hearing checks as appropriate
- Review of vaccine records for school requirements
- Completion of your school, camp or sports form
How to prepare
- Bring every form you need completed, with the parent section filled in.
- Bring the vaccine record and insurance card.
- Ask the school or team when the form is due. The fee for completing a school, camp or sports form is $25. Call ahead with the type of form and your deadline so we can tell you when it will be ready.
