The site uses one small cookie, jhp_lang, only to remember the language you choose. It is not used for tracking or advertising.

Our printable new-patient forms are PDF files that you print and fill out on paper. Nothing you write on them is sent through this website; bring the completed form to the office.

Like any website, our hosting provider may keep standard technical logs (such as IP address, browser type and pages requested) for security and to keep the site running. We do not use these logs for marketing.
