New patients

New Patients at Jackson Heights Pediatrics

We welcome new patients, from newborns to age 26. Here's how to get started.

How to become a patient

  1. Check your insurance. We accept insurance from 17 companies and programs, including NY Medicaid, and self-pay is available. See the full list on our insurance page. Plans change, so please call (917) 832-6562 to confirm your child's plan before the visit.
  2. Book the first visit. Call (917) 832-6562. Let us know it's your child's first visit.
  3. Fill out the new-patient form. Print it, fill it out and bring it to your visit: English (PDF) · Español (PDF). You can also fill one out in the office.
  4. Expecting a baby? We offer prenatal (meet-the-doctor) visits, so expecting parents can meet Dr. Ilyayeva before the baby is born. Call us to book one.
  5. Bring records. Bring your child's vaccine record and recent medical records, or ask your previous pediatrician to send them. Medical records are released in accordance with the law. Under HIPAA and New York law, we need a signed authorization from the parent or guardian, or from the patient if they are an adult. Call (917) 832-6562 to request records or to have records sent to us from another doctor.

What to bring to the first visit

  • Your child's insurance card and a parent or guardian's photo ID
  • Vaccine (immunization) records
  • Previous medical records, or your previous pediatrician's name and phone number
  • A list of any medicines, vitamins and allergies
  • Any school, daycare or camp forms that need to be filled out
  • Your questions. Writing them down ahead of time helps.

New babies

If you're expecting, plan your baby's first visit for 3 to 5 days after birth, within 2 to 3 days of leaving the hospital. Read about newborn care. Dr. Ilyayeva sees newborns in the office only; she does not visit babies in the hospital.
